Heat and weight work together here. A three-row SUV with the AC running flat out in August traffic asks a lot of the condenser, the compressor and the gearbox. The 5.3 and 6.2 V8s use cylinder deactivation, and the lifters behind it can fail, often starting as a tick at idle or a misfire. The 8-speed automatic in some models is known for a shudder at steady, light throttle.
Sand is the other factor. Desert weekends and dusty site roads clog air filters and pack the condenser fins, and plenty of Sierras work as trucks during the week. Give us the model year as well, because the engines and gearboxes changed between generations. Describe the symptom when you book and the diagnosis starts there.

On the 5.3 and 6.2 V8s, the lifters that switch cylinders off can collapse. A tick at idle, a misfire or a check-engine light is the early sign.
